Understanding Adrenal Masses: An Overview

Adrenal masses are abnormal growths that develop in the adrenal glands, which are small but vital endocrine organs located on top of each kidney. Each adrenal gland consists of two distinct regions: the cortex, which produces steroid hormones such as cortisol, aldosterone, and androgens, and the medulla, which produces catecholamines like adrenaline and noradrenaline. These hormones are essential for maintaining homeostasis, regulating blood pressure, controlling the body’s response to stress, and balancing metabolism and electrolytes. Adrenal masses may be identified during evaluation for hormone-related symptoms or discovered incidentally during imaging studies for unrelated conditions, a situation that has become increasingly common due to the widespread use of advanced imaging techniques such as CT and MRI. These incidental findings, often referred to as “adrenal incidentalomas,” present a clinical challenge because while many of these masses are benign and nonfunctional, a subset may be malignant or hormonally active and require further investigation and possible treatment. Understanding the nature of these masses is crucial for determining the appropriate clinical approach.
Adrenal masses can be broadly classified into functional and nonfunctional types. Functional adrenal tumors secrete excess hormones, leading to clinical syndromes such as Cushing’s syndrome (excess cortisol), Conn’s syndrome (excess aldosterone), or pheochromocytoma (excess catecholamines). Symptoms may include hypertension, weight gain, muscle weakness, palpitations, and anxiety. Nonfunctional tumors, on the other hand, do not produce hormones and are usually asymptomatic. Evaluation involves both imaging and hormonal testing. Computed tomography (CT) or magnetic resonance imaging (MRI) provides critical details about the size, shape, and internal characteristics of the mass. Concerning features such as irregular borders, size greater than 4 cm, and rapid growth can suggest malignancy. Simultaneously, blood and urine tests are used to measure hormone levels and assess whether the tumor is secreting cortisol, aldosterone, or catecholamines inappropriately.
Management of adrenal masses depends on their size, functional status, and imaging characteristics. Small, benign appearing, nonfunctional masses may be monitored with periodic imaging. However, larger masses (typically over 4 cm), those with suspicious imaging features, or hormonally active tumors often require surgical removal. Laparoscopic adrenalectomy is the standard approach for most benign tumors, while open surgery may be necessary for large or potentially malignant lesions. Adrenal cortical carcinoma is a rare but aggressive cancer that can present as a large adrenal mass. Early detection is critical, though prognosis tends to be poor in advanced stages. Metastatic disease from other primary cancers, such as lung or breast cancer, can also manifest as adrenal masses and must be carefully considered during evaluation. A structured diagnostic strategy, combining imaging and hormonal testing, is essential to determine the nature of the mass and guide appropriate treatment for the best possible patient outcomes.
